New Patient Information
Welcome to our practice, we look forward to meeting you and caring for your skin and eye health.
It would be very much appreciated if you could arrive 10 minutes prior to your initial consultation to complete our Patient Privacy & New Patient information sheet & Medical History form, this will ensure we can provide you with the best service possible. Alternatively, you can complete the forms prior to your appointment and bring them on the day, or forward them to the practice in advance for processing.
We request you bring the following to your appointment:
- Private Health Fund Details
- Medicare Card
- Pension / Health Care Card
- Veteran Affairs Card
- List of Current Medications
- Your Doctor Referral
At this appointment further treatment will be discussed and any necessary surgical appointments made.
Interpreters for non-English speaking patients can be arranged. Please advise us if an interpreter is required as soon as possible.
We would appreciate 24 hours notice if you are unable to keep your appointment. Failure to attend or advise that you are unable to keep this appointment may incur a non-attendance fee.
